Gynecological Care

Gynecological healthcare is a cornerstone of the BloomWell practice, and can last the entire span of a person's reproductive lifetime. First GYN appointments can be made in adolescence, and care can be continued until about ten years after your last menstrual cycle. At each appointment, we will collect a thorough history, provide a focused exam based on your needs and any symptoms you may be experiencing, and give you all the education and resources you need to make informed choices regarding your care.

GYN care includes:

• Preconception counseling
• Pelvic exams
• Pap smears
• STI counseling, screening, and treatment
• Infection screening and treatment
• Incontinence/urinary retention diagnosis and treatment
• Treatment of conditions affecting sexuality
• Pelvic floor health/prolapse assessment and counseling
• Urinary analysis and treatment for UTIs
• Hormone replacement therapy

Contraception

Contraceptive care is provided in combination with obstetric or gynecological services OR as a stand-alone service. Contraception appointments include a complete health history, a urinary and pregnancy screening, brief examination, and counseling regarding the most appropriate and safest choices for your individual situation and contraceptive wishes. A follow-up appointment is provided within three months to insure that everything is going well with your chosen method of contraception.

Contraception options include:

• Non-hormonal cycle awareness counseling
• Diaphragms
• Progestin-only pills
• Combined hormonal pills
• Injectables
• Implants--placement and removal (placement may require insurance preapproval and order of the device before placement; placement may take place at a subsequent visit).
• Intra-uterine devices--placement and removal (placement may require insurance preapproval and order of the device before placement; placement may take place at a subsequent visit).
• Intra-vaginal rings
• Referrals for permanent contraception, such as vasectomy or tubal ligation

Well-Woman Care

Well-Woman care at BloomWell encompasses the overall health of the whole woman; while acute primary care issues should be addressed by a primary healthcare provider, your midwife is able to provide a wide variety of preventative healthcare services. Annual Well-Woman appointments in conjunction with routine pelvic exams and Pap smears are encouraged to insure continuity of health.

Well-Woman Care includes:

• Breast cancer screenings
• Medication maintenance
• Mental health screenings and referrals
• Osteoporosis screenings
• Colon health screenings
• Diabetes screenings
• Nutritional and supplement counseling

Gynecological Service Fees

Cost for contraceptive device placement is for the visit only; cost for the device itself is additional and may be obtained with insurance coverage. For device placement, a GYN consult/new patient appointment is required prior to placement; only the returning patient appointment is charged for the placement visit. Removal of devices can occur free of charge during a regular GYN visit. 

THE ONE YEAR PACKAGE: $500
Includes:

• Initial intake/problem-focused appointment w/needed labs included if not an insurance recipient (pap smears not available for included labs but can be added on to this visit as needed).
• Prescription refills, medication management, medical notes, and specialist referrals as needed ($25 savings each)
• 4 complimentary telehealth appointments as needed within the calendar year of first appointment ($140 value)
• In person returning GYN/primary care visits for one year, as needed--$75 each ($25 savings per visit)
• Unlimited lab draws in office/lab orders as needed (lab processing fee separate) ($25 savings each)
• Unlimited messaging through the client portal
• $50 off DUTCH testing

AL LA CARTE: 

• GYN/primary care, new patient or returning patient after >1 year from first appointment: $250 
• Returning GYN/primary patient appointment w/in one year of first appointment: $100 each
• Telehealth: $35 each
• Lab orders/draws, specialist referrals, medical excuse notes, and prescription refills that are not a part of a paid appointment: $25 each
• Messaging in the client portal for questions/answers pertaining to the visit are included for the first 2 weeks after being seen; message conversations after two weeks may require a new follow up Telehealth or Returning visit.

DUTCH TESTING:

• DUTCH Complete only: $350
• DUTCH Complete w/visit to review results and develop plan of care: $400
• DUTCH Cycle Mapping only: $250
• DUTCH Cycle Mapping + Complete: $450
• DUTCH Cycle Mapping + Complete w/visit to review results and develop plan of care: $500

All patients are provided with detailed receipts. Superbills for insurance reimbursement are available upon request.
